I olika kliniska studier anv\u00e4ndes ett doseringsintervall fr\u00e5n 0,5 mg eller mindre upp till 25 mg eller mer, vilket g\u00f6r det sv\u00e5rt att j\u00e4mf\u00f6ra resultaten. Dessutom anv\u00e4ndes ocks\u00e5 olika formuleringar, s\u00e5som snabbfris\u00e4ttande eller kronobiologiskt kombinerat\/pulserande melatonin, s\u00e5 att en j\u00e4mf\u00f6relse inte heller h\u00e4r \u00e4r tydlig. Men om alla insamlade data hittills sammanfattas, kan f\u00f6ljande doseringsrekommendationer ses som l\u00e4mpliga: Under s\u00e5 kallad kronoterapi, som hj\u00e4lper till att \u00e5terst\u00e4lla den inre klockan, \u00e4r doseringsm\u00e4ngder p\u00e5 0,5 mg till 1 mg av en snabbfris\u00e4ttande formulering tillr\u00e4ckliga. En pulserande administrering p\u00e5 upp till 3 mg har ocks\u00e5 visat sig vara effektiv. Detta g\u00e4ller s\u00e4rskilt vid skiftarbete och\/eller jetlag. Tidpunkten f\u00f6r intag anpassas till den nya tidszonen f\u00f6r jetlag och till vilofasen f\u00f6r skiftarbetare.<\/p>\n<p>Om en klassisk substitutionsbehandling beh\u00f6vs, vilket \u00e4r fallet f\u00f6r \u00e4ldre, \u00e4r den pulserande doseringsformen l\u00e4mpligast snarare \u00e4n en l\u00e5ngsam eller snabb fris\u00e4ttande form. Det b\u00f6r s\u00e4kerst\u00e4llas att melatoninet tillf\u00f6rs kroppen i tillr\u00e4ckliga m\u00e4ngder under cirka sex till sju timmar under natten. Detta uppn\u00e5s genom en m\u00e4ngd p\u00e5 3 mg, som utan problem kan \u00f6kas upp till 6 mg om det beh\u00f6vs. Tidpunkten f\u00f6r intag \u00e4r cirka 30 minuter f\u00f6re s\u00e4ngg\u00e5endet, men alltid f\u00f6re midnatt f\u00f6r att undvika f\u00f6rskjutning av rytmen.<\/p>\n<p>St\u00f6djande behandlingar, t.ex. vid stroke eller hj\u00e4rtinfarkt, kommer s\u00e4kert att kr\u00e4va en h\u00f6gre dos. I inledande studier anv\u00e4ndes med framg\u00e5ng s\u00e5 h\u00f6ga doser som 50 mg som en omedelbar \u00e5tg\u00e4rd i form av snabbt fris\u00e4ttande melatonin. Administrering sker omedelbart efter k\u00e4nnedom om h\u00e4ndelsen, allts\u00e5 oavsett tid p\u00e5 dygnet. Medf\u00f6ljande terapier bland tum\u00f6rpatienter kr\u00e4ver ocks\u00e5 en h\u00f6gre dos. Det \u00e4r f\u00f6r n\u00e4rvarande fortfarande omtvistat om denna form endast ska administreras p\u00e5 kv\u00e4llen, men detta resultat \u00e4r mest troligt. N\u00e4r det g\u00e4ller str\u00e5lbehandling verkar en dos p\u00e5 15 mg vara tillr\u00e4cklig, vilket appliceras strax f\u00f6re str\u00e5lningen.<\/p>\n<p>Andra till\u00e4mpningar, t.ex. vid epilepsi, neuroprotektion och\/eller rytmstabilisering, gynnas b\u00e4st av en pulserande och\/eller f\u00f6rdr\u00f6jd doseringsform i en m\u00e4ngd av 3-6 mg p\u00e5 kv\u00e4llen. Det \u00e4r viktigt att notera att barn kr\u00e4ver en h\u00f6gre dos \u00e4n vuxna. \u00c4ven om det inte finns ett stort antal studier om biverkningar av melatonin, visar alla studier att melatonin \u00e4r mycket kompatibelt. En nyligen publicerad metaanalys av m\u00e5nga studier som mestadels administrerade \u00f6ver 10 mg visade att frekvensen och sv\u00e5righetsgraden av rapporterade biverkningar var identiska med de som intr\u00e4ffade hos patienter som fick placebo. Den f\u00f6rmodligen mest sannolika o\u00f6nskade biverkningen av melatonin upptr\u00e4der vid en administrering som \u00e4r felaktigt tidsinst\u00e4lld.
