I suoi quattro effetti principali sono la rigidit\u00e0 muscolare, i tremori, la bradicinesia (o lentezza dei movimenti) e la sensazione di instabilit\u00e0 in posizione eretta. Tuttavia, anche i disturbi del sonno e i disturbi emotivi sono estremamente comuni. In effetti, molte persone sviluppano i disturbi del sonno associati alla PD prima di avere altri sintomi della malattia.<\/p>\n<p>Le persone affette da PD di solito hanno difficolt\u00e0 ad addormentarsi e a mantenere il sonno. Il sonno REM \u00e8 particolarmente disturbato e si verifica troppo tardi nel ciclo del sonno o addirittura non si verifica affatto. Gran parte della stanchezza e dei disturbi emotivi di questa malattia possono essere dovuti ai disturbi del sonno che fanno parte della malattia di Parkinson.<\/p>\n<h2>La fisiopatologia dei disturbi del sonno correlati alla malattia di Parkinson<\/h2>\n<p>La melatonina \u00e8 il principale ormone associato al sonno. Secreta dalla ghiandola pineale su indicazione del nucleo soprachiasmatico del cervello, la melatonina si attacca ai recettori delle cellule e attiva o disattiva i geni dipendenti dal tempo. Mentre il nucleo soprachiasmatico non \u00e8 interessato dalla malattia di Parkinson, i recettori della melatonina sembrano diminuire in modo sostanziale nelle aree del cervello associate alla malattia di Parkinson. Ci\u00f2 impedisce alla melatonina di stimolare il sonno nelle persone affette da PD e lascia il ritmo circadiano senza una delle sue principali fonti di regolazione.<\/p>\n<h2>Come pu\u00f2 aiutare la melatonina a rilascio ritardato?<\/h2>\n<p>La melatonina \u00e8 stata a lungo utilizzata per trattare i disturbi del sonno. I ricercatori hanno scoperto che, mentre la melatonina a rilascio rapido \u00e8 pi\u00f9 indicata per i disturbi dell&#8217;inizio del sonno o per i disturbi che causano difficolt\u00e0 ad addormentarsi, non \u00e8 efficace per i disturbi del mantenimento del sonno. Gli inibitori delle COMT, ad esempio, sono una classe di farmaci comunemente utilizzata per il trattamento della malattia di Parkinson. Questi farmaci vengono assorbiti in livelli molto pi\u00f9 elevati se assunti al mattino, offrendo cos\u00ec un maggiore effetto terapeutico. Poich\u00e9 il trattamento della malattia di Parkinson \u00e8 complicato, \u00e8 importante assumere i farmaci nei momenti in cui sono pi\u00f9 efficaci.<\/p>\n<p>Gli studi nel campo della cronobiologia stanno offrendo nuovi trattamenti per alcune delle malattie pi\u00f9 difficili dell&#8217;umanit\u00e0.
