Les c\u00e9phal\u00e9es en grappe chroniques sont d\u00e9finies comme des crises r\u00e9currentes de c\u00e9phal\u00e9es en grappe pendant un an ou plus sans interruption ou avec de courtes interruptions sans sympt\u00f4mes pendant moins de trois mois. Il s&rsquo;agit de maux de t\u00eate brefs mais extr\u00eamement douloureux qui peuvent survenir pendant plusieurs jours, voire plusieurs semaines, d&rsquo;affil\u00e9e. Ces <a href=\"https:\/\/www.chronobiology.com\/the-hidden-link-between-sleep-and-headaches\/\" target=\"_blank\" rel=\"noopener\">maux de t\u00eate<\/a> peuvent durer de 15 minutes \u00e0 trois heures. Le mal de t\u00eate est similaire \u00e0 celui d&rsquo;une migraine, mais il y a quelques diff\u00e9rences essentielles. Contrairement aux migraines, qui peuvent durer une journ\u00e9e enti\u00e8re, voire plusieurs jours si elles ne sont pas trait\u00e9es, les c\u00e9phal\u00e9es en grappe durent g\u00e9n\u00e9ralement entre 15 et 180 minutes. S&rsquo;il est rare d&rsquo;avoir plus d&rsquo;une migraine par jour, il est possible d&rsquo;avoir jusqu&rsquo;\u00e0 huit c\u00e9phal\u00e9es en 24 heures. En outre, la douleur de la migraine peut varier localement. En revanche, les c\u00e9phal\u00e9es en grappe n&rsquo;affectent qu&rsquo;un seul c\u00f4t\u00e9 de la t\u00eate, g\u00e9n\u00e9ralement sur la tempe ou autour de l&rsquo;\u0153il. Enfin, les personnes souffrant de migraines ont tendance \u00e0 se reposer dans une pi\u00e8ce calme et sombre, alors que les personnes souffrant de c\u00e9phal\u00e9es en grappe sont plus susceptibles d&rsquo;\u00eatre agit\u00e9es et de faire les cent pas dans la pi\u00e8ce.<\/p>\n<p>Si les c\u00e9phal\u00e9es en grappe sont plus fr\u00e9quentes chez les hommes que chez les femmes, les recherches sugg\u00e8rent que le trouble est plus grave chez les femmes. Une <a href=\"https:\/\/n.neurology.org\/content\/100\/12\/547\" target=\"_blank\" rel=\"noopener\">\u00e9tude<\/a> portant sur 874 personnes et publi\u00e9e en ligne dans Neurology\u00ae a montr\u00e9 que les femmes \u00e9taient plus susceptibles d&rsquo;\u00eatre diagnostiqu\u00e9es avec des c\u00e9phal\u00e9es en grappe chroniques que les hommes. Les crises durent \u00e9galement plus longtemps chez les femmes que chez les hommes. Les femmes \u00e9taient \u00e9galement plus susceptibles de d\u00e9clarer que leurs crises se produisaient \u00e0 des moments diff\u00e9rents de la journ\u00e9e que <a href=\"https:\/\/www.chronobiology.com\/the-real-battle-of-the-sexes-sleep-differences-between-women-and-men\/\" target=\"_blank\" rel=\"noopener\">les hommes<\/a>.<\/p>\n<h2>Lien entre maux de t\u00eate et rythmes circadiens<\/h2>\n<p><a href=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2023\/04\/shutterstock_1306931836.jpg\"><img decoding=\"async\" class=\"alignright wp-image-45701 size-medium\" title=\"shutterstock 1306931836\" src=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2023\/04\/shutterstock_1306931836-300x212.jpg\" alt=\"shutterstock 1306931836\" width=\"300\" height=\"212\" srcset=\"https:\/\/dev.chronobiology.com\/wp-content\/uploads\/2023\/04\/shutterstock_1306931836-300x212.jpg 300w, https:\/\/dev.chronobiology.com\/wp-content\/uploads\/2023\/04\/shutterstock_1306931836-768x543.jpg 768w, https:\/\/dev.chronobiology.com\/wp-content\/uploads\/2023\/04\/shutterstock_1306931836.jpg 1000w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/a>Alors que des recherches ant\u00e9rieures ont mis en \u00e9vidence la <a href=\"https:\/\/www.chronobiology.com\/losing-sleep-is-worse-on-womens-memory-than-mens\/\" target=\"_blank\" rel=\"noopener\">diff\u00e9rence entre les sexes<\/a>, l&rsquo;\u00e9tude actuelle a \u00e9tabli un lien avec les rythmes circadiens. Il s&rsquo;agissait notamment d&rsquo;informations sur la chronologie des maux de t\u00eate au cours de la journ\u00e9e et de l&rsquo;ann\u00e9e, et d&rsquo;\u00e9tudes visant \u00e0 d\u00e9terminer si les g\u00e8nes associ\u00e9s \u00e0 l&rsquo;horloge circadienne sont plus fr\u00e9quents chez les personnes souffrant de ce type de maux de t\u00eate.<\/p>\n<p>Les chercheurs ont \u00e9galement examin\u00e9 des \u00e9tudes sur les c\u00e9phal\u00e9es en grappe et les migraines, ainsi que sur les hormones li\u00e9es au syst\u00e8me circadien, notamment le <a href=\"https:\/\/www.chronobiology.com\/cortisols-role-synchronization-circadian-rhythm\/\" target=\"_blank\" rel=\"noopener\">cortisol<\/a> et la m\u00e9latonine. Les donn\u00e9es sugg\u00e8rent que ces deux types de maux de t\u00eate sont fortement circadiens \u00e0 de multiples niveaux, en particulier les c\u00e9phal\u00e9es en grappe, ont d\u00e9clar\u00e9 les chercheurs. Cela souligne l&rsquo;importance de l&rsquo;hypothalamus &#8211; la r\u00e9gion du cerveau qui abrite l&rsquo;horloge biologique primaire &#8211; et son r\u00f4le dans les c\u00e9phal\u00e9es en grappe et les migraines. Cela soul\u00e8ve \u00e9galement des questions sur la g\u00e9n\u00e9tique des d\u00e9clencheurs, tels que les changements de sommeil, connus comme d\u00e9clencheurs de migraines, qui fournissent des indices sur le rythme circadien de l&rsquo;organisme.<\/p>\n<h3>Des niveaux de cortisol plus \u00e9lev\u00e9s et des niveaux de m\u00e9latonine plus faibles<\/h3>\n<p>En ce qui concerne les c\u00e9phal\u00e9es en grappe, la m\u00e9ta-analyse a mis en \u00e9vidence un sch\u00e9ma circadien des crises de maux de t\u00eate chez 71 % de l&rsquo;ensemble des personnes. Les crises atteignaient leur paroxysme aux heures tardives de la nuit et jusqu&rsquo;aux premi\u00e8res heures du matin. Au fil de l&rsquo;ann\u00e9e, les crises \u00e9taient plus nombreuses au printemps et \u00e0 l&rsquo;automne. Au niveau g\u00e9n\u00e9tique, les c\u00e9phal\u00e9es en grappe ont \u00e9t\u00e9 associ\u00e9es \u00e0 deux g\u00e8nes circadiens majeurs, et cinq des neuf g\u00e8nes qui augmentent la probabilit\u00e9 de c\u00e9phal\u00e9es en grappe sont des g\u00e8nes dont l&rsquo;expression est circadienne. Les personnes souffrant de c\u00e9phal\u00e9es en grappe pr\u00e9sentaient \u00e9galement des taux de cortisol plus \u00e9lev\u00e9s et des taux de m\u00e9latonine plus faibles que les personnes ne souffrant pas de c\u00e9phal\u00e9es en grappe.<\/p>\n<p>En ce qui concerne les<a href=\"https:\/\/www.chronobiology.com\/sleep-and-migraine-could-fragmented-sleep-cause-migraine-days-later\/\" target=\"_blank\" rel=\"noopener\"> migraines<\/a>, la m\u00e9ta-analyse a montr\u00e9 un sch\u00e9ma de crise circadien chez 50 % des personnes. Alors que le pic de crises pendant la journ\u00e9e \u00e9tait g\u00e9n\u00e9ralis\u00e9, allant de la fin de la matin\u00e9e au d\u00e9but de la soir\u00e9e, il y avait un creux circadien pendant la nuit, lorsque peu de crises se produisaient. La migraine \u00e9tait \u00e9galement associ\u00e9e \u00e0 deux g\u00e8nes circadiens fondamentaux, et 110 des 168 g\u00e8nes associ\u00e9s \u00e0 la migraine \u00e9taient des g\u00e8nes \u00e0 expression circadienne. Les personnes souffrant de migraines pr\u00e9sentaient des taux de m\u00e9latonine plus faibles dans leurs urines que celles qui n&rsquo;en souffraient pas. En outre, les <a href=\"https:\/\/www.chronobiology.com\/home-lighting-suppresses-melatonin-levels-impacting-sleep\/\" target=\"_blank\" rel=\"noopener\">niveaux de m\u00e9latonine<\/a> \u00e9taient plus faibles pendant une crise de migraine.<\/p>\n<h2>Nouvelles options th\u00e9rapeutiques<\/h2>\n<p>Selon les chercheurs, ces r\u00e9sultats augmentent le potentiel d&rsquo;utilisation des <a href=\"https:\/\/www.chronobiology.com\/discovery-that-circadian-rhythm-regulates-blood-brain-barrier-may-lead-to-improved-treatment-options\/\" target=\"_blank\" rel=\"noopener\">traitements circadiens<\/a> pour les maux de t\u00eate.
