L&rsquo;\u00e9tude, publi\u00e9e dans <a href=\"https:\/\/www.thelancet.com\/journals\/lanpsy\/article\/PIIS2215-0366(18)30139-1\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\"><em>The Lancet Psychiatry<\/em><\/a>, sugg\u00e8re que les <a href=\"https:\/\/www.chronobiology.com\/circadian-sleep-disorders\/\" target=\"_blank\" rel=\"noopener noreferrer\">perturbations de nos rythmes circadiens naturels<\/a> peuvent entra\u00eener un d\u00e9clin cognitif et un risque accru de troubles mentaux tels que les troubles bipolaires et les troubles d\u00e9pressifs majeurs.<\/p>\n<h2>Les rythmes circadiens et leur impact sur la sant\u00e9 et la maladie<\/h2>\n<p>Toutes les formes de vie expos\u00e9es \u00e0 la lumi\u00e8re du soleil poss\u00e8dent des dispositifs inn\u00e9s de synchronisation que nous appelons rythmes circadiens. Ces horloges naturelles contr\u00f4lent pratiquement tous les processus biologiques de notre corps et tendent \u00e0 suivre un cycle typique de 24 heures. Essentiels \u00e0 notre bien-\u00eatre g\u00e9n\u00e9ral, les rythmes circadiens jouent un r\u00f4le cl\u00e9 dans la r\u00e9gulation des fonctions corporelles telles que la r\u00e9g\u00e9n\u00e9ration cellulaire, la production d&rsquo;hormones, les processus m\u00e9taboliques, l&rsquo;immunit\u00e9 et m\u00eame les r\u00e9ponses au stress.<\/p>\n<p>Ce que l&rsquo;on pourrait consid\u00e9rer comme l'\u00a0\u00bbhorloge ma\u00eetresse\u00a0\u00bb de l&rsquo;organisme se trouve dans une partie du cerveau situ\u00e9e dans l&rsquo;hypothalamus et appel\u00e9e <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3609582\/\" target=\"_blank\" rel=\"noopener noreferrer\">noyau suprachiasmatique<\/a>. Le noyau suprachiasmatique utilise des signaux lumineux pour synchroniser les nombreuses horloges auxiliaires qui r\u00e9sident dans l&rsquo;ensemble du corps. Presque toutes les cellules de notre corps poss\u00e8dent un groupe unique de prot\u00e9ines qui s&rsquo;activent ou se d\u00e9sactivent en fonction de l&rsquo;heure de la journ\u00e9e. Des facteurs tels que le travail post\u00e9, les modes de vie en int\u00e9rieur et m\u00eame les voyages \u00e0 travers les fuseaux horaires peuvent tous perturber les d\u00e9licats m\u00e9canismes de l&rsquo;horloge photosensible, d\u00e9clenchant une longue s\u00e9rie d&rsquo;effets secondaires ind\u00e9sirables.<\/p>\n<p>La recherche a toujours montr\u00e9 que m\u00eame de petites perturbations de ces rythmes ont des effets n\u00e9fastes sur la sant\u00e9. Il est largement admis que le manque de sommeil provoque des <a href=\"http:\/\/www.pnas.org\/content\/108\/4\/1657\" target=\"_blank\" rel=\"noopener noreferrer\">troubles cognitifs<\/a> et de l&rsquo;irritabilit\u00e9, mais il a \u00e9galement \u00e9t\u00e9 d\u00e9montr\u00e9 que les perturbations du sommeil augmentent le risque de <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4370346\/\" target=\"_blank\" rel=\"noopener noreferrer\">maladies cardiaques, d&rsquo;ob\u00e9sit\u00e9 et de diab\u00e8te de type 2<\/a>. Cette nouvelle \u00e9tude vient s&rsquo;ajouter aux preuves de plus en plus nombreuses que les rythmes irr\u00e9guliers de sommeil et d&rsquo;\u00e9veil sont \u00e9troitement li\u00e9s \u00e0 des probl\u00e8mes de sant\u00e9 mentale tels que la d\u00e9pression, l&rsquo;anxi\u00e9t\u00e9 et un <a href=\"https:\/\/www.chronobiology.com\/the-missing-link-between-mood-sleep-the-per3-gene\/\" target=\"_blank\" rel=\"noopener noreferrer\">sentiment g\u00e9n\u00e9ral de malaise<\/a>.<\/p>\n<h2>Une \u00e9tude \u00e0 grande \u00e9chelle r\u00e9v\u00e8le un lien significatif entre les perturbations du sommeil et de l&rsquo;\u00e9veil et l&rsquo;humeur<\/h2>\n<p>Bien que des \u00e9tudes ant\u00e9rieures aient sugg\u00e9r\u00e9 l&rsquo;existence d&rsquo;un lien entre les perturbations du sommeil et de l&rsquo;\u00e9veil et la sant\u00e9 mentale, la recherche n&rsquo;a pas \u00e9t\u00e9 concluante en raison de l&rsquo;autod\u00e9claration, de la petite taille des \u00e9chantillons et de l&rsquo;absence d&rsquo;ajustement pour les facteurs de confusion appropri\u00e9s. Dans l&rsquo;espoir de r\u00e9soudre ces probl\u00e8mes de m\u00e9thodologie, des chercheurs de l&rsquo;universit\u00e9 de Glasgow ont analys\u00e9 les donn\u00e9es d&rsquo;activit\u00e9 de plus de 91 000 participants dans le cadre de ce qui est la plus grande \u00e9tude d&rsquo;observation de ce type \u00e0 \u00e9tablir un lien significatif entre les horloges internes du corps, le bien-\u00eatre mental et les effets n\u00e9fastes sur la sant\u00e9.<\/p>\n<p>L&rsquo;\u00e9quipe a analys\u00e9 les donn\u00e9es d&rsquo;activit\u00e9 de 91 105 personnes issues de la <a href=\"http:\/\/www.ukbiobank.ac.uk\/\" target=\"_blank\" rel=\"noopener noreferrer\">UK Biobank,<\/a> un projet de recherche national en cours qui suit la sant\u00e9 d&rsquo;un demi-million de r\u00e9sidents britanniques, afin d&rsquo;obtenir une mesure objective des sch\u00e9mas de repos et d&rsquo;activit\u00e9, ce qu&rsquo;ils appellent l&rsquo;amplitude relative. Les participants s\u00e9lectionn\u00e9s \u00e9taient \u00e2g\u00e9s de 37 \u00e0 73 ans et devaient porter un acc\u00e9l\u00e9rom\u00e8tre pendant sept jours entre 2013 et 2015 afin d&rsquo;enregistrer leur activit\u00e9. Les informations relatives \u00e0 leur activit\u00e9 ont \u00e9t\u00e9 associ\u00e9es \u00e0 des questionnaires sur la sant\u00e9 mentale qui ont \u00e9t\u00e9 utilis\u00e9s pour \u00e9valuer les sympt\u00f4mes de sant\u00e9 mentale, les sentiments de bien-\u00eatre subjectif ainsi que les fonctions cognitives.<\/p>\n<p>En construisant des mod\u00e8les math\u00e9matiques, l&rsquo;\u00e9quipe a \u00e9tudi\u00e9 les associations entre une faible amplitude relative, d\u00e9finie par une activit\u00e9 accrue pendant les p\u00e9riodes de repos ou une activit\u00e9 r\u00e9duite pendant la journ\u00e9e, et la fonction cognitive, le bien-\u00eatre g\u00e9n\u00e9ral ainsi que le risque de d\u00e9velopper un trouble de l&rsquo;humeur au cours de la vie. D&rsquo;apr\u00e8s leurs donn\u00e9es, la perturbation circadienne est fortement associ\u00e9e \u00e0<\/p>\n<ul>\n<li>un risque accru de <a href=\"https:\/\/www.chronobiology.com\/predicting-depression-severity-using-melatonin-samples\/\" target=\"_blank\" rel=\"noopener noreferrer\">trouble d\u00e9pressif majeur<\/a><\/li>\n<li>un risque accru de trouble bipolaire<\/li>\n<li>l&rsquo;instabilit\u00e9 de l&rsquo;humeur<\/li>\n<li>des scores de neuroticisme plus \u00e9lev\u00e9s<\/li>\n<li>une augmentation du sentiment de solitude<\/li>\n<li>diminution du bonheur<\/li>\n<li>diminution de la satisfaction \u00e0 l&rsquo;\u00e9gard de la sant\u00e9<\/li>\n<li>un ralentissement des temps de r\u00e9action (un indicateur indirect de la fonction cognitive).<\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2018\/06\/strong-link-uncovered-between-body-clock-and-mental-health-2.jpg\"><img decoding=\"async\" class=\"size-medium wp-image-22004 alignright\" src=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2018\/06\/strong-link-uncovered-between-body-clock-and-mental-health-2-288x300.jpg\" alt=\"Strong Link Uncovered Between Body Clock and Mental Health\" width=\"288\" height=\"300\" srcset=\"https:\/\/dev.chronobiology.com\/wp-content\/uploads\/2018\/06\/strong-link-uncovered-between-body-clock-and-mental-health-2-288x300.jpg 288w, https:\/\/dev.chronobiology.com\/wp-content\/uploads\/2018\/06\/strong-link-uncovered-between-body-clock-and-mental-health-2-768x800.jpg 768w, https:\/\/dev.chronobiology.com\/wp-content\/uploads\/2018\/06\/strong-link-uncovered-between-body-clock-and-mental-health-2.jpg 800w\" sizes=\"(max-width: 288px) 100vw, 288px\" \/><\/a>L&rsquo;\u00e9quipe a constat\u00e9 que ces associations \u00e9taient ind\u00e9pendantes des facteurs de confusion possibles tels que la d\u00e9mographie, l&rsquo;\u00e9ducation ou le mode de vie.<\/p>\n<p><a href=\"https:\/\/www.thelancet.com\/doi\/story\/10.1016\/audio.2018.05.14.107047\" target=\"_blank\" rel=\"noopener noreferrer\">Laura Lyall,<\/a> auteur de l&rsquo;\u00e9tude, a d\u00e9clar\u00e9 dans un <a href=\"https:\/\/www.eurekalert.org\/pub_releases\/2018-05\/tl-tlp051418.php\" target=\"_blank\" rel=\"noopener noreferrer\">communiqu\u00e9 de presse<\/a>: \u00ab\u00a0Nos r\u00e9sultats indiquent une association entre l&rsquo;alt\u00e9ration des rythmes circadiens quotidiens et les troubles de l&rsquo;humeur et du bien-\u00eatre. Cependant, il s&rsquo;agit d&rsquo;associations observationnelles qui ne nous permettent pas de savoir si les troubles de l&rsquo;humeur et la diminution du bien-\u00eatre sont dus \u00e0 une perturbation des sch\u00e9mas repos-activit\u00e9 ou si une rythmicit\u00e9 circadienne perturb\u00e9e rend les personnes vuln\u00e9rables aux troubles de l&rsquo;humeur et \u00e0 une diminution du bien-\u00eatre\u00a0\u00bb.<\/p>\n<p>Elle ajoute que \u00ab\u00a0pour examiner cette question plus en d\u00e9tail, il sera utile, dans le cadre de futures \u00e9tudes, de suivre les sch\u00e9mas de repos-activit\u00e9 des participants au fil du temps afin de voir si des rythmes perturb\u00e9s peuvent pr\u00e9dire si une personne est plus susceptible de d\u00e9velopper un trouble de l&rsquo;humeur\u00a0\u00bb.<\/p>\n<p>L&rsquo;\u00e9quipe reconna\u00eet qu&rsquo;il existe encore des limites \u00e0 leur recherche, en notant que les maladies mentales commencent g\u00e9n\u00e9ralement dans un segment de population beaucoup plus jeune que celui qui \u00e9tait repr\u00e9sent\u00e9 ici. Les auteurs notent que \u00ab\u00a0l&rsquo;amplitude relative est relativement peu co\u00fbteuse et facile \u00e0 mesurer et pourrait \u00eatre utilis\u00e9e pour identifier les personnes pr\u00e9sentant un risque plus \u00e9lev\u00e9 de trouble d\u00e9pressif majeur ou de trouble bipolaire, ou celles qui pourraient b\u00e9n\u00e9ficier de traitements ciblant directement l&rsquo;horloge circadienne\u00a0\u00bb. \u00c9tant donn\u00e9 que la plupart des troubles mentaux apparaissent au cours de l&rsquo;adolescence, des \u00e9tudes longitudinales sur des populations plus jeunes pourraient am\u00e9liorer notre compr\u00e9hension des m\u00e9canismes de causalit\u00e9 et nous aider \u00e0 trouver de nouveaux moyens de pr\u00e9dire les troubles de l&rsquo;humeur et d&rsquo;affiner les traitements\u00a0\u00bb.<\/p>\n<p>Il reste difficile de d\u00e9terminer si les perturbations de l&rsquo;horloge entra\u00eenent des probl\u00e8mes de sant\u00e9 ou si c&rsquo;est en fait l&rsquo;inverse.<\/p>\n<p>\u00ab\u00a0Bien que nos r\u00e9sultats ne puissent pas nous renseigner sur le sens de la causalit\u00e9, ils renforcent l&rsquo;id\u00e9e que les troubles de l&rsquo;humeur sont associ\u00e9s \u00e0 des rythmes circadiens perturb\u00e9s, et nous apportons la preuve que l&rsquo;alt\u00e9ration des rythmes repos-activit\u00e9 est \u00e9galement li\u00e9e \u00e0 une d\u00e9t\u00e9rioration du bien-\u00eatre subjectif et des capacit\u00e9s cognitives\u00a0\u00bb, a d\u00e9clar\u00e9 le Dr Lyall.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Une \u00e9tude \u00e0 grande \u00e9chelle men\u00e9e au Royaume-Uni r\u00e9v\u00e8le des liens \u00e9troits entre les perturbations du sommeil et de l&rsquo;\u00e9veil et les troubles de l&rsquo;humeur.
