De nombreuses personnes aux premiers stades de la d\u00e9mence ne parviennent plus \u00e0 dormir la nuit. En cons\u00e9quence, elles peuvent \u00eatre somnolentes pendant la journ\u00e9e. Certaines deviennent tr\u00e8s agit\u00e9es en d\u00e9but de soir\u00e9e ou en fin d&rsquo;apr\u00e8s-midi (\u00ab\u00a0sundowning\u00a0\u00bb). Au fur et \u00e0 mesure que la maladie progresse, les patients deviennent plus \u00e9veill\u00e9s la nuit et dorment davantage le jour, parfois au point d&rsquo;inverser compl\u00e8tement le cycle veille-sommeil normal de l&rsquo;\u00eatre humain. La gestion de ces perturbations du sommeil est l&rsquo;un des aspects les plus difficiles pour les aidants et l&rsquo;une des principales raisons pour lesquelles les patients atteints de la maladie d&rsquo;Alzheimer sont plac\u00e9s en institution.<\/p>\n<h2>Pourquoi des perturbations du sommeil ?<\/h2>\n<p>Le noyau suprachiasmatique hypothalamique (NSC) est consid\u00e9r\u00e9 comme l&rsquo;horloge ma\u00eetresse du corps humain. Il se trouve dans le cerveau, pr\u00e8s de l&rsquo;endroit o\u00f9 les nerfs optiques se croisent. Il re\u00e7oit des informations des yeux sur les niveaux de luminosit\u00e9. Lorsqu&rsquo;il d\u00e9tecte une faible luminosit\u00e9, il d\u00e9clenche la s\u00e9cr\u00e9tion de <a href=\"https:\/\/www.chronobiology.com\/sleep-2\/melatonin\/\" target=\"_blank\" rel=\"noopener\">m\u00e9latonine<\/a> par la glande pin\u00e9ale, une hormone qui provoque le sommeil et aide \u00e0 coordonner les <a href=\"https:\/\/www.chronobiology.com\/about-chronobiology\/#CircadianRhythm\" target=\"_blank\" rel=\"noopener\">rythmes circadiens<\/a> du corps. De nombreuses fonctions de l&rsquo;organisme suivent des rythmes circadiens. Le plus \u00e9vident est le cycle veille-sommeil. Les autopsies de patients atteints de la maladie d&rsquo;Alzheimer r\u00e9v\u00e8lent g\u00e9n\u00e9ralement une d\u00e9g\u00e9n\u00e9rescence importante du SCN aux stades avanc\u00e9s de la maladie. Cependant, m\u00eame aux premiers stades de la maladie d&rsquo;Alzheimer, on observe un dysfonctionnement \u00e9vident du SCN et une perturbation de la production rythmique de m\u00e9latonine.<\/p>\n<h2>La perturbation du sommeil est-elle \u00e0 l&rsquo;origine de la maladie d&rsquo;Alzheimer ?<\/h2>\n<p><a href=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2015\/01\/the-science-of-chronobiology-sheds-light-on-treatments-for-sleeping-disorders-in-alzheimers-3.jpg\"><img decoding=\"async\" class=\"alignright size-medium wp-image-5602\" src=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2015\/01\/the-science-of-chronobiology-sheds-light-on-treatments-for-sleeping-disorders-in-alzheimers-3-300x246.jpg\" alt=\"Chronobiology, Sleeping Disorders, Alzheimer's \" width=\"300\" height=\"246\" srcset=\"https:\/\/www.chronobiology.com\/wp-content\/uploads\/2015\/01\/the-science-of-chronobiology-sheds-light-on-treatments-for-sleeping-disorders-in-alzheimers-3-300x246.jpg 300w, https:\/\/www.chronobiology.com\/wp-content\/uploads\/2015\/01\/the-science-of-chronobiology-sheds-light-on-treatments-for-sleeping-disorders-in-alzheimers-3-768x629.jpg 768w, https:\/\/www.chronobiology.com\/wp-content\/uploads\/2015\/01\/the-science-of-chronobiology-sheds-light-on-treatments-for-sleeping-disorders-in-alzheimers-3.jpg 800w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/a>Certains professionnels de la sant\u00e9 ont \u00e9mis l&rsquo;hypoth\u00e8se que la perturbation du sommeil est soit le premier sympt\u00f4me de la maladie d&rsquo;Alzheimer, soit la <a href=\"http:\/\/www.medicalnewstoday.com\/articles\/274188.php\" target=\"_blank\" rel=\"noopener\">cause m\u00eame de la maladie d&rsquo;Alzheimer<\/a>. Ces hypoth\u00e8ses sont fond\u00e9es sur des \u00e9tudes d&rsquo;observation chez l&rsquo;homme et sur des \u00e9tudes exp\u00e9rimentales chez l&rsquo;animal. Les personnes qui souffrent de troubles du sommeil \u00e0 l&rsquo;\u00e2ge de 70 ans courent un risque beaucoup plus \u00e9lev\u00e9 de d\u00e9velopper la maladie d&rsquo;Alzheimer \u00e0 l&rsquo;\u00e2ge de 90 ans. Il est bien \u00e9tabli que les troubles du sommeil chez l&rsquo;homme entra\u00eenent un d\u00e9clin cognitif et des d\u00e9ficits de m\u00e9moire, deux ph\u00e9nom\u00e8nes qui s&rsquo;inversent lorsque les personnes rattrapent leur retard de sommeil. Si le d\u00e9ficit de sommeil ne peut \u00eatre invers\u00e9, il peut devenir permanent ou m\u00eame \u00e9voluer vers la d\u00e9mence et la neurod\u00e9g\u00e9n\u00e9rescence, c&rsquo;est-\u00e0-dire la maladie d&rsquo;Alzheimer. Un mod\u00e8le murin de <a href=\"https:\/\/www.chronobiology.com\/chronobiology-circadian-rhythms-and-the-science-of-sleep\/\" target=\"_blank\" rel=\"noopener\">privation<\/a> chronique <a href=\"https:\/\/www.chronobiology.com\/chronobiology-circadian-rhythms-and-the-science-of-sleep\/\" target=\"_blank\" rel=\"noopener\">de sommeil<\/a> a r\u00e9v\u00e9l\u00e9 que les souris pr\u00e9sentaient des troubles de la m\u00e9moire et d\u00e9veloppaient dans leur cerveau des enchev\u00eatrements et des plaques qui ressemblent \u00e0 ceux que l&rsquo;on observe dans la maladie d&rsquo;Alzheimer. Cette d\u00e9couverte a incit\u00e9 certains chercheurs \u00e0 \u00e9mettre l&rsquo;hypoth\u00e8se que le traitement des troubles du sommeil observ\u00e9s dans les premiers stades de la maladie d&rsquo;Alzheimer pourrait peut-\u00eatre inverser ou au moins ralentir l&rsquo;\u00e9volution de la maladie. Le traitement des troubles du sommeil faciliterait certainement la prise en charge des patients atteints de la maladie d&rsquo;Alzheimer.<\/p>\n<h2>La luminoth\u00e9rapie ?<\/h2>\n<p>Les premi\u00e8res \u00e9tudes sur l&rsquo;utilisation de la <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3158242\/\" target=\"_blank\" rel=\"noopener\">luminoth\u00e9rapie<\/a> pour traiter les patients n&rsquo;ont pas \u00e9t\u00e9 tr\u00e8s concluantes, mais apr\u00e8s la mise en place d&rsquo;une m\u00e9thode statistiquement plus correcte pour analyser les donn\u00e9es des \u00e9tudes d&rsquo;activit\u00e9, le domaine est all\u00e9 de l&rsquo;avant. L&rsquo;exposition des patients atteints de la maladie d&rsquo;Alzheimer \u00e0 une lumi\u00e8re intense tout au long de la matin\u00e9e a permis d&rsquo;am\u00e9liorer les cycles normaux du sommeil ainsi que l&rsquo;humeur et le comportement des patients. Toutefois, il a fallu des mois de traitement quotidien pour que les patients obtiennent une r\u00e9ponse claire. Les adultes en bonne sant\u00e9 souffrant de troubles du sommeil r\u00e9agissent \u00e0 une luminoth\u00e9rapie similaire en deux semaines ou moins. Les chercheurs ont \u00e9mis l&rsquo;hypoth\u00e8se que le manque de sensibilit\u00e9 \u00e0 la lumi\u00e8re pourrait \u00eatre en partie \u00e0 l&rsquo;origine des troubles du sommeil observ\u00e9s dans la maladie d&rsquo;Alzheimer. Fait int\u00e9ressant, dans une \u00e9tude publi\u00e9e dans <em>Chronobiology International,<\/em> les chercheurs ont fix\u00e9 des capteurs sur des patients atteints de la maladie d&rsquo;Alzheimer et ont simplement enregistr\u00e9 la quantit\u00e9 de lumi\u00e8re \u00e0 laquelle ils \u00e9taient expos\u00e9s et leur degr\u00e9 d&rsquo;activit\u00e9. Ils ont constat\u00e9 que la plupart des patients \u00e9taient expos\u00e9s \u00e0 des niveaux continus de lumi\u00e8re tr\u00e8s faible, sans changement apparent au cours de la journ\u00e9e et de la nuit. Certains patients \u00e9taient expos\u00e9s \u00e0 plus de lumi\u00e8re la nuit que le jour, et ces patients avaient tendance \u00e0 ne pas pr\u00e9senter de sch\u00e9ma circadien dans leurs p\u00e9riodes d&rsquo;agitation. Il est peu probable que ce type de conditions dans les institutions aide les patients \u00e0 retrouver des rythmes circadiens. Pensant que le fait d&rsquo;imposer aux patients atteints de la maladie d&rsquo;Alzheimer une perception plus structur\u00e9e de la journ\u00e9e pourrait les aider \u00e0 r\u00e9tablir une meilleure perception du jour et de la nuit, un groupe a men\u00e9 un essai contr\u00f4l\u00e9 randomis\u00e9 comparant l&rsquo;absence d&rsquo;intervention \u00e0 une combinaison de luminoth\u00e9rapie matinale et de marche en plein air pendant 30 minutes chaque jour. On pensait que la luminoth\u00e9rapie matinale, l&rsquo;exposition quotidienne \u00e0 la lumi\u00e8re du jour et la pratique d&rsquo;une activit\u00e9 physique saine contribueraient \u00e0 normaliser les rythmes circadiens des patients. Si le SCN ne r\u00e9agit que lentement \u00e0 la lumi\u00e8re chez les patients atteints de la maladie d&rsquo;Alzheimer, l&rsquo;administration directe de m\u00e9latonine aux patients pourrait peut-\u00eatre faire l&rsquo;affaire.<\/p>\n<h2>Lumi\u00e8re vive et m\u00e9latonine<\/h2>\n<p>L&rsquo;\u00e9tape suivante consiste \u00e9videmment \u00e0 combiner la luminoth\u00e9rapie et la m\u00e9latonine. Une <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2642966\/\" target=\"_blank\" rel=\"noopener\">\u00e9tude<\/a> combinant la luminoth\u00e9rapie le matin et la m\u00e9latonine orale le soir a montr\u00e9 que cette combinaison \u00e9tait raisonnablement efficace pour am\u00e9liorer l&rsquo;\u00e9veil et l&rsquo;activit\u00e9 diurnes chez les patients Alzheimer institutionnalis\u00e9s. La prochaine \u00e9tape consistera \u00e0 essayer ce traitement combin\u00e9 sur des patients atteints de la maladie d&rsquo;Alzheimer \u00e0 un stade pr\u00e9coce et vivant encore chez eux.
