Forskellige kliniske studier brugte et doseringsomr\u00e5de fra 0,5 mg eller mindre op til 25 mg eller mere, hvilket g\u00f8r det vanskeligt at sammenligne resultaterne. Derudover blev der ogs\u00e5 brugt forskellige formuleringer, s\u00e5som hurtigt frigivende eller kronobiologisk kombineret\/pulserende melatonin, s\u00e5 en sammenligning er heller ikke klar her. Men hvis alle de indsamlede data til dato opsummeres, kan de f\u00f8lgende doseringsanbefalinger ses som passende: Under s\u00e5kaldt kronoterapi, som hj\u00e6lper med at nulstille det indre ur, er doseringsm\u00e6ngder p\u00e5 0,5 mg til 1 mg af en hurtigt frig\u00f8rende formulering tilstr\u00e6kkelige. En pulserende administration p\u00e5 op til 3 mg har ogs\u00e5 vist sig at v\u00e6re effektiv. Det g\u00e6lder is\u00e6r ved skifteholdsarbejde og\/eller jetlag. Indtagelsestidspunktet passer til den nye tidszone for jetlag og til hvilefasen for skifteholdsarbejdere.<\/p>\n<p>Hvis der er behov for en klassisk substitutionsbehandling, som det er tilf\u00e6ldet for \u00e6ldre, er den pulserende doseringsform mest hensigtsm\u00e6ssig frem for en langsom eller hurtig frig\u00f8rende form. Det skal sikres, at kroppen f\u00e5r tilstr\u00e6kkelige m\u00e6ngder melatonin i ca. seks til syv timer i l\u00f8bet af natten. Dette opn\u00e5s gennem en m\u00e6ngde p\u00e5 3 mg, som uden problemer kan \u00f8ges op til 6 mg, hvis det er n\u00f8dvendigt. Indtagelsestidspunktet er ca. 30 minutter f\u00f8r sengetid, men altid f\u00f8r midnat for at undg\u00e5 at forskyde rytmen.<\/p>\n<p>Underst\u00f8ttende behandlinger, f.eks. i tilf\u00e6lde af slagtilf\u00e6lde eller hjerteanfald, vil helt sikkert kr\u00e6ve en h\u00f8jere dosis. I de f\u00f8rste unders\u00f8gelser blev der med succes anvendt s\u00e5 store m\u00e6ngder som 50 mg som en \u00f8jeblikkelig foranstaltning i form af hurtigt frig\u00f8rende melatonin. Administrationen f\u00f8lger umiddelbart efter kendskab til begivenheden, s\u00e5 uanset tidspunktet p\u00e5 dagen. Ledsagende behandlinger blandt tumorpatienter kr\u00e6ver ogs\u00e5 en h\u00f8jere dosis. I \u00f8jeblikket diskuteres det stadig, om denne form kun skal administreres om aftenen, men dette resultat er mest sandsynligt. I tilf\u00e6lde af str\u00e5lebehandling ser en dosis p\u00e5 15 mg ud til at v\u00e6re tilstr\u00e6kkelig, hvilket anvendes kort f\u00f8r str\u00e5ling.<\/p>\n<p>Andre anvendelser s\u00e5som epilepsi, neurobeskyttelse og\/eller rytmestabilisering profiterer bedst af en pulserende og\/eller forsinket doseringsform i en m\u00e6ngde p\u00e5 3-6 mg om aftenen. Det er vigtigt at bem\u00e6rke, at b\u00f8rn kr\u00e6ver en h\u00f8jere dosis end voksne. Selvom der ikke findes mange unders\u00f8gelser af bivirkninger ved melatonin, viser alle unders\u00f8gelser, at melatonin er meget foreneligt. En nyligt offentliggjort metaanalyse af talrige unders\u00f8gelser, der for det meste administrerede over 10 mg, viste, at hyppigheden og sv\u00e6rhedsgraden af rapporterede bivirkninger var identisk med dem, der opstod hos patienter, der fik placebo. Den formodentlig mest sandsynlige u\u00f8nskede bivirkning af melatonin vil opst\u00e5 ved en indgivelse, der er forkert timet.
