B\u011bhem posledn\u00edch 20 let bylo v mnoha klinick\u00fdch studi\u00edch testov\u00e1no a prok\u00e1z\u00e1no terapeutick\u00e9 vyu\u017eit\u00ed melatoninu v r\u016fzn\u00fdch oblastech medic\u00edny a to, \u017ee u\u017e\u00edv\u00e1n\u00ed tohoto hormonu m\u016f\u017ee b\u00fdt \u00fa\u010dinnou l\u00e9\u010dbou r\u016fzn\u00fdch onemocn\u011bn\u00ed. V budoucnu bude prov\u00e1d\u011bno je\u0161t\u011b v\u00edce v\u00fdzkum\u016f o nejr\u016fzn\u011bj\u0161\u00edch mo\u017enostech vyu\u017eit\u00ed melatoninu, zejm\u00e9na ve spojen\u00ed s jin\u00fdmi tradi\u010dn\u00edmi l\u00e9\u010debn\u00fdmi postupy, a to bu\u010f za \u00fa\u010delem zv\u00fd\u0161en\u00ed jejich terapeutick\u00e9 \u00fa\u010dinnosti, nebo sn\u00ed\u017een\u00ed jejich toxicity. V\u011bt\u0161ina zde vyprodukovan\u00e9ho melatoninu se v\u0161ak neuvol\u0148uje do krve, ale vyu\u017e\u00edv\u00e1 se p\u0159edev\u0161\u00edm p\u0159\u00edmo na m\u00edst\u011b.<\/p>\n<p>P\u0159esto\u017ee p\u0159esn\u00e1 funkce melatoninu v tr\u00e1vic\u00ed soustav\u011b nen\u00ed dosud zn\u00e1ma, m\u00e1 tento hormon d\u00edky sv\u00fdm antioxida\u010dn\u00edm vlastnostem ochrannou funkci proti vzniku v\u0159ed\u016f v \u017ealude\u010dn\u00ed sliznici. Zda hraj\u00ed roli mechanismy \u00fa\u010dinku p\u0159en\u00e1\u0161en\u00e9 receptory, se v sou\u010dasn\u00e9 dob\u011b st\u00e1le d\u016fkladn\u011b zkoum\u00e1.<\/p>\n<h3 style=\"text-align: center;\">Gastroezofage\u00e1ln\u00ed refluxn\u00ed choroba<\/h3>\n<p>Mnoho hormon\u016f, kter\u00e9 souvisej\u00ed s j\u00eddeln\u00edm chov\u00e1n\u00edm a tr\u00e1ven\u00edm, jako jsou ghrelin, inzulin a leptin, se pravideln\u011b uvol\u0148uj\u00ed ve velmi specifick\u00fdch \u010dasech cyklicky po dobu 24 hodin. M\u00edsto cyklick\u00e9ho uvol\u0148ov\u00e1n\u00ed hormon\u016f, nap\u0159\u00edklad kdy\u017e je \u010dlov\u011bk vzh\u016fru a mus\u00ed j\u00edst, se nyn\u00ed hormony uvol\u0148uj\u00ed n\u00e1hodn\u011b. Proto\u017ee tyto hormony mohou b\u00fdt bez p\u0159\u00edtomnosti potravy v tr\u00e1vic\u00edm traktu \u0161kodliv\u00e9, proto\u017ee zp\u016fsobuj\u00ed dodate\u010dnou produkci \u017ealude\u010dn\u00ed kyseliny. Z toho lze vyvodit, \u017ee to m\u016f\u017ee b\u00fdt hlavn\u00ed p\u0159\u00ed\u010dinou mnoha onemocn\u011bn\u00ed za\u017e\u00edvac\u00edho traktu a poruch p\u0159\u00edjmu potravy.<\/p>\n<p>Nedostate\u010dn\u00e1 hormon\u00e1ln\u00ed regulace spolu s m\u011bn\u00edc\u00edmi se cykly autonomn\u00edch \u010dinnost\u00ed m\u016f\u017ee v\u00e9st k poruch\u00e1m tr\u00e1ven\u00ed. V p\u0159\u00edpad\u011b gastroezofage\u00e1ln\u00ed refluxn\u00ed choroby, kdy se nespr\u00e1vn\u011b zv\u00fd\u0161en\u00e9 mno\u017estv\u00ed \u017ealude\u010dn\u00ed kyseliny tla\u010d\u00ed do j\u00edcnu, m\u016f\u017ee melatonin skute\u010dn\u011b sn\u00ed\u017eit po\u0161kozen\u00ed oxidovan\u00e9 DNA sliznice. Krom\u011b toho m\u016f\u017ee hormon slou\u017eit jako ochrana \u017ealude\u010dn\u00ed sliznice p\u0159ed zdrav\u00ed ohro\u017euj\u00edc\u00edmi l\u00e1tkami, jako jsou salicyl\u00e1ty (obsa\u017een\u00e9 v potravin\u00e1ch a l\u00e9c\u00edch proti bolesti), a\/nebo vrtuln\u00edkov\u00fdmi bakteriemi.<\/p>\n<h3 style=\"text-align: center;\">Syndrom dr\u00e1\u017ediv\u00e9ho tra\u010dn\u00edku<\/h3>\n<p>Syndrom dr\u00e1\u017ediv\u00e9ho tra\u010dn\u00edku (IBS) je charakterizov\u00e1n nevysv\u011btlitelnou bolest\u00ed \u017ealudku, plynatost\u00ed spojenou se zm\u011bnami ve vyprazd\u0148ov\u00e1n\u00ed, jako je z\u00e1cpa nebo pr\u016fjem, a celkov\u00fdm nepohodl\u00edm. V tomto p\u0159\u00edpad\u011b m\u016f\u017ee melatonin pomoci prost\u0159ednictv\u00edm regulace motility a citlivosti tr\u00e1vic\u00edho traktu. R\u016fzn\u00e9 studie prok\u00e1zaly, \u017ee melatonin m\u00e1 pozitivn\u00ed \u00fa\u010dinek p\u0159i l\u00e9\u010db\u011b syndromu dr\u00e1\u017ediv\u00e9ho tra\u010dn\u00edku mimo jin\u00e9 t\u00edm, \u017ee sni\u017euje bolesti \u017ealudku a kone\u010dn\u00edku a sni\u017euje souvisej\u00edc\u00ed poruchy sp\u00e1nku. Nov\u00e9 studie nav\u00edc prok\u00e1zaly, \u017ee by tento hormon mohl hr\u00e1t d\u016fle\u017eitou roli tak\u00e9 se zdrav\u00edm st\u0159evn\u00ed fl\u00f3ry a jej\u00edm cirkadi\u00e1nn\u00edm rytmem. Tr\u00e1vic\u00ed bakterie maj\u00ed sv\u016fj vlastn\u00ed cirkadi\u00e1nn\u00ed rytmus, kter\u00fd ovliv\u0148uje i na\u0161i funk\u010dnost. St\u00e1le v\u00edce studi\u00ed nazna\u010duje, \u017ee tito mikrobi jsou pro lidsk\u00e9 zdrav\u00ed nesm\u00edrn\u011b d\u016fle\u017eit\u00ed a \u017ee i mal\u00e1 nerovnov\u00e1ha m\u016f\u017ee zp\u016fsobit poruchu. Pr\u00e1v\u011b tento rytmus tr\u00e1vic\u00edch bakteri\u00ed je pomoc\u00ed melatoninu synchronizov\u00e1n s cirkadi\u00e1nn\u00edmi procesy na\u0161eho t\u011bla.<\/p>\n<h3 style=\"text-align: center;\">Z\u00e1n\u011btliv\u00e1 st\u0159evn\u00ed onemocn\u011bn\u00ed<\/h3>\n<p>Pat\u0159\u00ed sem \u0159ada chronick\u00fdch z\u00e1n\u011btliv\u00fdch onemocn\u011bn\u00ed, jako je ulcer\u00f3zn\u00ed kolitida (chronick\u00fd z\u00e1n\u011bt tlust\u00e9ho st\u0159eva) a Crohnova choroba. Tento hormon sni\u017euje ostatn\u00ed stresov\u00e9 hormony, sni\u017euje krevn\u00ed tlak a dosahuje pozitivn\u00edho vlivu na kardiovaskul\u00e1rn\u00ed syst\u00e9m. Melatonin m\u016f\u017ee b\u00fdt u\u017eite\u010dn\u00fd p\u0159i sni\u017eov\u00e1n\u00ed srde\u010dn\u00ed hypertrofie a sni\u017eovat \u010detnost srde\u010dn\u00edch infarkt\u016f. Hormon p\u016fsob\u00ed tak\u00e9 jako odp\u016frce voln\u00fdch radik\u00e1l\u016f, kter\u00e9 napadaj\u00ed srde\u010dn\u00ed sval. Vzhledem k tomu, \u017ee melatonin m\u00e1 protiz\u00e1n\u011btliv\u00e9 vlastnosti, m\u016f\u017ee b\u00fdt \u00fa\u010dinn\u011b vyu\u017e\u00edv\u00e1n tak\u00e9 p\u0159i l\u00e9\u010db\u011b arterioskler\u00f3zy. N\u011bkolik invitro studi\u00ed prok\u00e1zalo, \u017ee melatonin \u00fa\u010dinn\u011b inhibuje oxidaci LDL, kter\u00e1 je jednou z hlavn\u00edch p\u0159\u00ed\u010din vzniku arterioskler\u00f3zy.<\/p>\n<h3 style=\"text-align: center;\">Vysok\u00fd krevn\u00ed tlak<\/h3>\n<p>Vysok\u00fd krevn\u00ed tlak pat\u0159\u00ed k onemocn\u011bn\u00edm roz\u0161\u00ed\u0159en\u00fdm v z\u00e1padn\u00edch pr\u016fmyslov\u00fdch zem\u00edch. Melatonin se uk\u00e1zal jako u\u017eite\u010dn\u00fd p\u0159i regulaci krevn\u00edho tlaku. Studie dok\u00e1zaly, \u017ee peror\u00e1ln\u00ed u\u017e\u00edv\u00e1n\u00ed melatoninu m\u016f\u017ee sn\u00ed\u017eit vysok\u00fd krevn\u00ed tlak i reaktivitu c\u00e9v. Je to d\u016fsledek toho, \u017ee melatonin m\u00e1 uvol\u0148uj\u00edc\u00ed \u00fa\u010dinek na c\u00e9vy a p\u016fsob\u00ed jako siln\u00fd lapa\u010d radik\u00e1l\u016f, proto\u017ee voln\u00e9 radik\u00e1ly maj\u00ed negativn\u00ed vliv na krevn\u00ed tlak. Nicm\u00e9n\u011b p\u0159esn\u00e9 mechanismy p\u016fsoben\u00ed melatoninu s ohledem na krevn\u00ed tlak nejsou dodnes zn\u00e1my.<\/p>\n<h3 style=\"text-align: center;\">Srde\u010dn\u00ed infarkt a mrtvice<\/h3>\n<p>Srde\u010dn\u00ed infarkt i mozkov\u00e1 mrtvice se t\u00fdkaj\u00ed akutn\u00edho uz\u00e1v\u011bru c\u00e9vy, tak\u017ee tk\u00e1n\u011b, kter\u00e9 jsou touto c\u00e9vou z\u00e1sobov\u00e1ny, jsou zbaveny kysl\u00edku a po\u0161kozeny. Hlavn\u00edm c\u00edlem l\u00e9\u010dby je co nejrychleji obnovit z\u00e1sobov\u00e1n\u00ed krv\u00ed a kysl\u00edkem. Jakmile se obnov\u00ed \u017eivotod\u00e1rn\u00e9 z\u00e1sobov\u00e1n\u00ed kysl\u00edkem, je ji\u017e po\u0161kozen\u00e1 tk\u00e1\u0148 n\u00e1chyln\u00e1 k dal\u0161\u00edmu oxida\u010dn\u00edmu po\u0161kozen\u00ed v d\u016fsledku z\u00e1plavy p\u0159ich\u00e1zej\u00edc\u00edho kysl\u00edku. P\u0159esn\u011bji \u0159e\u010deno, tato sekund\u00e1rn\u00ed po\u0161kozen\u00ed lze sn\u00ed\u017eit nebo jim dokonce zcela zabr\u00e1nit sou\u010dasn\u00fdm pod\u00e1v\u00e1n\u00edm vy\u0161\u0161\u00ed d\u00e1vky melatoninu. V\u011bdci p\u0159edpokl\u00e1daj\u00ed, \u017ee mutace v genu pro melatoninov\u00fd receptor MT2 naru\u0161uje spojen\u00ed mezi vnit\u0159n\u00edmi hodinami a uvol\u0148ov\u00e1n\u00edm inzulinu. To m\u016f\u017ee v\u00e9st k abnorm\u00e1ln\u00edm hodnot\u00e1m cukru v krvi a nakonec k cukrovce. Asi t\u0159etina lid\u00ed m\u00e1 pon\u011bkud pozm\u011bn\u011bnou verzi tohoto melatoninov\u00e9ho receptoru, kter\u00fd se nach\u00e1z\u00ed na bun\u011b\u010dn\u00fdch membr\u00e1n\u00e1ch beta bun\u011bk ve slinivce b\u0159i\u0161n\u00ed. Beta bu\u0148ky ve slinivce b\u0159i\u0161n\u00ed jsou zodpov\u011bdn\u00e9 za uvol\u0148ov\u00e1n\u00ed inzul\u00ednu. Pokud se melatonin nav\u00e1\u017ee na receptory t\u011bchto bun\u011bk, uvol\u0148ov\u00e1n\u00ed se zpomal\u00ed. M\u00edrn\u011b mutovan\u00e1 verze beta bun\u011bk v\u0161ak na melatonin reaguje zv\u00fd\u0161en\u00edm po\u010dtu receptor\u016f. T\u00edm se zes\u00edl\u00ed \u00fa\u010dinek, kter\u00fd melatonin na bu\u0148ky m\u00e1, co\u017e zp\u016fsobuje hypoglyk\u00e9mii a dal\u0161\u00ed p\u0159eddiabetick\u00e9 stavy. Kdy\u017e lid\u00e9 s touto genovou variantou pracuj\u00ed a jed\u00ed v noci, kdy je jejich hladina melatoninu vysok\u00e1, pak maj\u00ed vy\u0161\u0161\u00ed hladinu cukru v krvi, kter\u00e1 zvy\u0161uje riziko vzniku cukrovky a dal\u0161\u00edch endokrinn\u00edch onemocn\u011bn\u00ed.<\/p>\n<p>R\u016fzn\u00e9 studie prok\u00e1zaly, \u017ee melatonin m\u016f\u017ee ovliv\u0148ovat sekreci inzulinu a homeost\u00e1zu gluk\u00f3zy. Pokusy na bu\u0148k\u00e1ch vedly ke zji\u0161t\u011bn\u00ed, \u017ee tento hormon potla\u010duje uvol\u0148ov\u00e1n\u00ed inzulinu v beta bu\u0148k\u00e1ch slinivky b\u0159i\u0161n\u00ed. Krysy, kter\u00e9 m\u011bly sklon k diabetu 2. typu, byly u\u017e\u00edv\u00e1n\u00edm melatoninu chr\u00e1n\u011bny p\u0159ed hyperlipidemi\u00ed a hyperglyk\u00e9mi\u00ed. U my\u0161\u00ed do\u0161lo ke zlep\u0161en\u00ed inzulinov\u00e9 rezistence i metabolismu gluk\u00f3zy. V n\u00e1hodn\u00e9 dvojit\u011b zaslepen\u00e9 studii s 22 \u017eenami po menopauze bylo zji\u0161t\u011bno, \u017ee melatonin zlep\u0161uje gluk\u00f3zovou toleranci a citlivost na inzulin u star\u0161\u00edch \u017een. Jin\u00e1 studie zjistila, \u017ee melatonin je \u00fa\u010dinn\u00fdm antioxidantem, kter\u00fd zlep\u0161uje \u00fa\u010dinnost peror\u00e1ln\u00edch antidiabetik. Pozitivn\u00ed v\u00fdsledky lze nav\u00edc pozorovat u pacient\u016f, kte\u0159\u00ed trp\u00ed sez\u00f3nn\u00ed depres\u00ed. Jin\u00e1 studie uv\u00e1d\u00ed ni\u017e\u0161\u00ed m\u00edru deprese u pacient\u016f se zimn\u00ed depres\u00ed, kte\u0159\u00ed byli l\u00e9\u010deni n\u00edzkou d\u00e1vkou melatoninu v odpoledn\u00edch hodin\u00e1ch. K r\u00e1nu pak hladina op\u011bt rychle kles\u00e1, aby umo\u017enila probuzen\u00ed. T\u00edm se napodobuj\u00ed zdrav\u00e9, norm\u00e1ln\u00ed cykly cirkadi\u00e1nn\u00edho rytmu, kter\u00e9 jsou spojeny s klidn\u00fdm sp\u00e1nkem.<\/p>\n<p>V\u00fdzkum v oblasti chronobiologie uk\u00e1zal, \u017ee udr\u017eov\u00e1n\u00ed melatoninov\u00e9ho cyklu je d\u016fle\u017eit\u00e9 nejen pro klidn\u00fd sp\u00e1nek, ale tak\u00e9 pro celkovou pohodu. Z tohoto d\u016fvodu st\u00e1le v\u00edce odborn\u00edk\u016f na zdrav\u00ed doporu\u010duje jako ide\u00e1ln\u00ed dopln\u011bk stravy dob\u0159e na\u010dasovan\u00e9 uvol\u0148ov\u00e1n\u00ed melatoninu. V takov\u00e9m p\u0159\u00edpad\u011b p\u0159ich\u00e1z\u00ed v \u00favahu pou\u017eit\u00ed jak dob\u0159e na\u010dasovan\u00e9ho p\u0159\u00edpravku s melatoninem, tak p\u0159\u00edpravku s rychle se uvol\u0148uj\u00edc\u00edm melatoninem, aby se p\u0159eprogramoval p\u0159irozen\u00fd cirkadi\u00e1nn\u00ed rytmus na po\u017eadovan\u00fd no\u010dn\u00ed rytmus.
