M\u016f\u017ee b\u00fdt tak\u00e9 zhor\u0161ena reak\u010dn\u00ed doba. V nejhor\u0161\u00edch p\u0159\u00edpadech se mohou objevit kardiovaskul\u00e1rn\u00ed onemocn\u011bn\u00ed, cukrovka a rakovina.<\/p>\n<p>V tomto p\u0159\u00edpad\u011b je melatonin \u00fa\u010dinn\u00fd p\u0159i regulaci posunu rytmu sp\u00e1nek-bd\u011bn\u00ed, jak k nim doch\u00e1z\u00ed, zejm\u00e9na p\u0159i letu p\u0159es v\u00edce \u010dasov\u00fdch p\u00e1sem. \u010cetn\u00e9 studie prok\u00e1zaly, \u017ee melatonin m\u016f\u017ee urychlit p\u0159izp\u016fsoben\u00ed se nov\u00e9mu \u010dasov\u00e9mu p\u00e1smu. Melatonin se rovn\u011b\u017e osv\u011bd\u010dil p\u0159i zm\u00edr\u0148ov\u00e1n\u00ed dal\u0161\u00edch p\u0159\u00edznak\u016f jetlagu. Melatonin je tak\u00e9 vhodn\u00fd pro pracovn\u00edky na sm\u011bny, kte\u0159\u00ed \u010dasto trp\u00ed poruchami sp\u00e1nku, proto\u017ee tito lid\u00e9 &#8211; podobn\u011b jako lid\u00e9 se syndromem jetlag &#8211; jsou vystaveni \u010dast\u00fdm zm\u011bn\u00e1m denn\u00edho rytmu. Je d\u016fle\u017eit\u00e9 db\u00e1t na p\u0159esn\u00e9 na\u010dasov\u00e1n\u00ed p\u0159\u00edjmu, aby byl posun f\u00e1z\u00ed zp\u016fsoben\u00fd melatoninem \u00fa\u010dinn\u00fd. P\u0159\u00edkladem takov\u00e9ho ide\u00e1ln\u00edho \u010dasu m\u016f\u017ee b\u00fdt nap\u0159\u00edklad doba mezi 21. a 22. hodinou nov\u00e9ho \u010dasov\u00e9ho p\u00e1sma u osob s jetlagem a jedna hodina p\u0159ed novou f\u00e1z\u00ed odpo\u010dinku u pracovn\u00edk\u016f na sm\u011bny. Bylo prok\u00e1z\u00e1no, \u017ee n\u011bkter\u00e9 l\u00e9ky na span\u00ed, jako jsou benzodiazepiny, a tak\u00e9 srde\u010dn\u00ed p\u0159\u00edpravky, jako jsou beta-blok\u00e1tory, potla\u010duj\u00ed tvorbu melatoninu. Nadm\u011brn\u00e1 konzumace alkoholu a\/nebo kofeinu z k\u00e1vy nebo \u010dern\u00e9ho \u010daje m\u016f\u017ee m\u00edt rovn\u011b\u017e negativn\u00ed vliv na uvol\u0148ov\u00e1n\u00ed melatoninu a v\u00e9st k naru\u0161en\u00ed sp\u00e1nku nebo nezdrav\u00e9mu sp\u00e1nku. P\u0159i snaze podpo\u0159it endogenn\u00ed produkci melatoninu u\u017e\u00edv\u00e1n\u00edm p\u0159edb\u011b\u017en\u00fdch l\u00e1tek, jako je tryptofan a\/nebo 5HTP, je d\u016fle\u017eit\u00e9 si uv\u011bdomit, \u017ee star\u0161\u00ed lid\u00e9 ji\u017e nejsou schopni syntetizovat melatonin z v\u00fd\u0161e uveden\u00fdch l\u00e1tek. Obvykle se objevuje u dosp\u00edvaj\u00edc\u00edch a mlad\u00fdch dosp\u011bl\u00fdch, kte\u0159\u00ed pat\u0159\u00ed k chronotypu &#8222;no\u010dn\u00ed sovy&#8220;. Z noci si d\u011blaj\u00ed den a z\u016fst\u00e1vaj\u00ed vzh\u016fru a\u017e do \u010dasn\u00fdch rann\u00edch hodin a pak sp\u00ed a\u017e do odpoledne. Posti\u017een\u00ed uv\u00e1d\u011bj\u00ed chronickou nespavost.<\/p>\n<p>Syndrom opo\u017ed\u011bn\u00e9ho n\u00e1stupu sp\u00e1nku t\u011bchto sov se nejsn\u00e1ze l\u00e9\u010d\u00ed pomoc\u00ed rann\u00ed sv\u011bteln\u00e9 terapie. Doporu\u010duje se prov\u00e1d\u011bt sv\u011btelnou terapii brzy r\u00e1no, aby se rytmus sp\u00e1nku a bd\u011bn\u00ed posunul dop\u0159edu. Podobn\u00fdch \u00fa\u010dink\u016f lze dos\u00e1hnout u\u017e\u00edv\u00e1n\u00edm melatoninu v prvn\u00edch ve\u010dern\u00edch hodin\u00e1ch, a\u0165 u\u017e se jedn\u00e1 o melatonin rychle se uvol\u0148uj\u00edc\u00ed nebo pulzuj\u00edc\u00ed. Mo\u017en\u00e1 je tak\u00e9 kombinace obou terapi\u00ed.<\/p>\n<h3 style=\"text-align: center;\">Poruchy sp\u00e1nku a bd\u011bn\u00ed u nevidom\u00fdch (syndrom Non-24)<\/h3>\n<p>Non-24 je z\u00e1va\u017en\u00e1, z\u00e1va\u017en\u00e1 a chronick\u00e1 porucha rytmu den\/noc, kter\u00e1 je zvl\u00e1\u0161t\u011b \u010dast\u00e1 u nevidom\u00fdch. Lid\u00e9 se syndromem non-24 nejsou schopni p\u0159izp\u016fsobit nebo synchronizovat sv\u00e9 vnit\u0159n\u00ed hodiny 24hodinov\u00e9mu denn\u00edmu rytmu. Tito lid\u00e9 \u017eij\u00ed ve sv\u00e9m vlastn\u00edm rytmu, kter\u00fd trv\u00e1 mezi 24 a p\u016fl a 25 hodinami. To vede k tomu, \u017ee ka\u017ed\u00fd den us\u00ednaj\u00ed a vst\u00e1vaj\u00ed o 30-60 minut pozd\u011bji. Podle toho se posouvaj\u00ed i v\u0161echny ostatn\u00ed rytmy t\u011bla, jako je t\u011blesn\u00e1 teplota, uvol\u0148ov\u00e1n\u00ed hormon\u016f nebo v\u00fd\u0161ka aktivity.<\/p>\n<p>C\u00edlem chronobiologicky spr\u00e1vn\u00e9 terapie je synchronizovat vnit\u0159n\u00ed hodiny posti\u017een\u00fdch lid\u00ed s 24hodinov\u00fdm denn\u00edm rytmem. Pokud je to nutn\u00e9, lze toho dos\u00e1hnout r\u00e1no pomoc\u00ed sv\u011bteln\u00e9 terapie a tak\u00e9 pomoc\u00ed l\u00e1tek, kter\u00e9 potla\u010duj\u00ed denn\u00ed ospalost. To v\u0161ak funguje pouze u t\u011bch, kte\u0159\u00ed jsou je\u0161t\u011b schopni minim\u00e1ln\u011b vn\u00edmat sv\u011btlo a tmu. U v\u0161ech ostatn\u00edch pacient\u016f by m\u011blo no\u010dn\u00ed pod\u00e1v\u00e1n\u00ed melatoninu nebo podobn\u011b p\u016fsob\u00edc\u00edch l\u00e1tek podpo\u0159it sp\u00e1nek a slou\u017eit jako \u010dasom\u00edra, podle kter\u00e9 se vnit\u0159n\u00ed hodiny \u0159\u00edd\u00ed.<\/p>\n<p>K \u00faprav\u011b endogenn\u00edho rytmu nevidom\u00fdch na rytmus norm\u00e1ln\u00edho dne a noci lze pou\u017e\u00edt r\u016fzn\u00e9 l\u00e1tky: Na jedn\u00e9 stran\u011b je mo\u017en\u00e9 pou\u017e\u00edt melatonin jako hormon, kter\u00fd ve\u010der ud\u00e1v\u00e1 \u010das. Po\u017eit\u00edm melatoninu, a\u0165 u\u017e se jedn\u00e1 o rychle se uvol\u0148uj\u00edc\u00ed nebo pulzuj\u00edc\u00ed prepar\u00e1t, je t\u011blu signalizov\u00e1no, \u017ee je nyn\u00ed noc. V posledn\u00ed dob\u011b jsou k dispozici tak\u00e9 n\u011bkter\u00e9 l\u00e1tky podobn\u00e9 melatoninu, nap\u0159\u00edklad Tasimelteon (Hetlioz\u00ae), kter\u00fd je rovn\u011b\u017e t\u0159eba u\u017e\u00edvat ve\u010der a p\u016fsob\u00ed p\u0159es melatoninov\u00e9 receptory v mozku na nastaven\u00ed vnit\u0159n\u00edch hodin podle rytmu den\/noc.<\/p>\n<h3 style=\"text-align: center;\">Poruchy sp\u00e1nku ve st\u00e1\u0159\u00ed a ve zvl\u00e1\u0161tn\u00edch \u017eivotn\u00edch situac\u00edch<\/h3>\n<p>P\u0159esto\u017ee u lid\u00ed zat\u00edm nebyl prok\u00e1z\u00e1n \u00fa\u010dinek prodlu\u017euj\u00edc\u00ed \u017eivot, je jist\u00e9, \u017ee melatonin m\u00e1 pozitivn\u00ed vliv na kvalitu \u017eivota, zejm\u00e9na u star\u0161\u00edch lid\u00ed. P\u0159edpokl\u00e1d\u00e1 se tak\u00e9, \u017ee hormon pom\u00e1h\u00e1 odd\u00e1lit molekul\u00e1rn\u00ed destrukci, \u00fabytek bun\u011bk a po\u0161kozen\u00ed organismu, co\u017e jsou faktory, kter\u00e9 p\u0159isp\u00edvaj\u00ed ke st\u00e1rnut\u00ed.<\/p>\n<p>V d\u016fsledku s v\u011bkem souvisej\u00edc\u00edho poklesu no\u010dn\u00ed hladiny melatoninu trp\u00ed star\u0161\u00ed lid\u00e9 \u010dasto poruchami sp\u00e1nku. V sou\u010dasn\u00e9 dob\u011b se p\u0159edpokl\u00e1d\u00e1, \u017ee ka\u017ed\u00fd druh\u00fd \u010dlov\u011bk star\u0161\u00ed 65 let trp\u00ed poruchami sp\u00e1nku. V tomto p\u0159\u00edpad\u011b se osv\u011bd\u010dila l\u00e9\u010dba melatoninem. Studie prok\u00e1zaly, \u017ee u\u017e\u00edv\u00e1n\u00ed melatoninu zaji\u0161\u0165uje nejen zlep\u0161en\u00ed kvality sp\u00e1nku, ale tak\u00e9 sn\u00ed\u017een\u00ed depres\u00ed a obav. Nicm\u00e9n\u011b studie s \u017eenami v perimenopauze dok\u00e1zaly, \u017ee \u010dinnost hypof\u00fdzy i funkce \u0161t\u00edtn\u00e9 \u017el\u00e1zy se mohou zlep\u0161it po\u017eit\u00edm melatoninu. Na druhou stranu deprese, kter\u00e1 nen\u00ed sez\u00f3nn\u011b charakterizov\u00e1na, je doprov\u00e1zena sp\u00ed\u0161e ztr\u00e1tou chuti k j\u00eddlu, \u00fabytkem hmotnosti a zkr\u00e1cen\u00edm sp\u00e1nku.
